Facility:Dayton Children's - Main CampusDepartment:PFK Care CoordinationSchedule:Full timeHours:40Job Details:The BSW Care Coordination Medical Social Worker assists patients and families with complex medical and behavioral health needs to navigate uneventfully, efficiently, and as effectively as possible through the course of their illness to improve or maintain social, emotional, functional, and physical health. Practice professional social work and promotes patient advocacy in accordance with the NASW Code of Ethics for Social Work. These activities are performed in a variety of settings including during inpatient stays, outpatient specialty clinic visits, emergency room visits, home, and community visits, as well as telephone and “virtual” visits.

Bachelors in Social work with LSW license required
Full time, 40 hours
M -F, 8-430
Serving the Endocrinology clinic
No weekends or holidays

LSW Ohio - Ohio Licensed Social Worker - Counselor, Social Worker and Marriage & Family Therapist Board
