The Salvation Army Support Services Case Manager in Dayton, Ohio

Overview

DEPARTMENT: Worship & ArtsREPORTS TO: Manger of Support Services & Corps MinistriesSUPERVISES: N/aSTATUS: Non-ExemptSCHEDULE: Full TimeFUNCTION: This person will meet with individuals/families who are seeking financial support given by The Salvation Army in Dayton. Through an assessment process, they will evaluate the need to determine The Salvation Army response. Pathway of Hope will be implemented through this process by the case manager. POH provides targeted services for families with a desire to take action to break the cycle of crisis and build a path out of intergenerational poverty. With the guidance of the divisional POH regional coordinator, the case manager will use a strengths-based, Stages of Change approach and Motivational interviewing skills to help families set and achieve goals that will bring about a better future for their families. They will provide excellent service to all who participate in functions held at the Kroc Center. In conjunction, they will keep as a priority, the promoting of the Christian faith and values in keeping with the Mission of The Salvation Army.

Responsibilities

DUTIES:Case Management includes:• Assessing client eligibility and providing assistance as warranted• Developing plans to improve a client’s well being• Helping clients adjust to changes in their lives such as the loss of a job or loved one• Helping clients work with government agencies from which they are applying for benefits• Responding to crisis situations• Maintaining complete and accurate client records and documentationThe general duties for Networking include:• Becoming familiar with services in the community• Networking with other service agencies as needed

• Serve as Salvation Army Liaison to other community based social services within Montgomery County

Pathway of Hope Case Management:• Provide services to the client family via the case management steps

  1. Identify, assess and enroll clients per POH Eligibility Criteria procedure2. Meet with the client for 1-2 hours per session, weekly or biweekly over 6-18 months to follow the goal plan3. Meet and assess upon case Management Exit and post-Exit follow-up sessions, per procedures4. Introduce Pastoral Care that is available to the family at the Corps; follow procedures if the family is interested5. Assist and teach within the case manager’s scope and refer out when appropriate, for life skill development

o Includes job readiness and job search, educational advancement, parenting, budgeting and finances, cooking and nutrition, conflict management, substance abuse awareness, and self-advocacy

  1. Ensure clients have access to health and human services

o Includes all Salvation Army internal provisions and services that are availableo Includes external agency referrals for housing, health care, mental health services, legal, childcare, non-cash benefits, and other resources as needed

  1. Enable clients to enhance their social support systems

o Including community activities and organizations, their children’s schools, and faith communities

  1. Work with the Kroc Corps Officer or Corps Mentors for the families9. Work with the Manager of Support Services & Corps Ministry on Quarterly Families Activities for POH Families along with Corp Mentors

Christmas Assistance:• Assisting with Christmas registration• Assisting with data entry• Clearing out duplications of address and household members• Handling client phone calls and requests regarding distribution• Participating in distribution

Life Skills:• Coordinating life skills programs throughout the Kroc Center• Securing volunteers and/or other agencies to teach various life skills at the Kroc Center• Working with events, marketing and front desk mangers to communicate the details of these programs• Keeping attendance at these programs and reporting them monthly to Manger of Support Services & Corps MinistriesKroc Social Work Liaison:• Assisting Kroc employees when issues involving apparent social work needs arise among Kroc program participants• Coordinating Support Groups with a focus on spiritual component• This includes mandated reporting of suspected abuse of children or vulnerable adults• Additionally, the Social Work Liaison takes social work concerns off the plate of employees who find themselves involved in situations in which they are not trained to deal with

Gift of Power program:• Participating in Gift of Power program (Jan-Apr).• Answering phones, making appointments, assessing clients’ eligibility, making pledges• Assisting Manager of Support Services & Corps Ministries to maintain accurate records• Participating in meetings with AES when asked by Manger of Support Services & Corps Ministries or Associate Kroc Center Administrator

Any other duties deemed necessary by the Kroc Center Administrators

Qualifications

MINIMUM QUALIFICATIONS:• Must have a Bachelor of Science in Social Work, or other related field.• Preferred licensed clinical social worker (LCSW) in the State of Ohio or interest an in pursuing license.• Ideal candidate would be fluent / proficient / conversant in Spanish• Strong creative and communication skills• Case management experience• Administrative skills• Experience in networking with others• Able to multi-task and meet hard deadlines• Able to work in team environment
