The Nurse Practitioner (NP) for Transitional Care Unit (TCU) practices under the general supervision of Pulmonary and Gastroenterology physician groups. The NP serves as an expert clinical nurse practitioner who utilizes specialized knowledge and skill obtained from advanced formal education and clinical expertise to promote optimum care of patients and families with acute and chronic pediatric issues. The NP in TCU assesses the health status of the pediatric client through interview, health history, physical examinations, and diagnostic tests. In collaboration with the physician, the NP evaluates test findings, makes initial medical diagnosis, provides patient/family education, and initiates appropriate action to facilitate the implementation of the therapeutic plan consistent with the continuing health care needs of the patient. The NP will participate in the provision of health care services for pediatric patients with acute and chronic illness.

Services include, but are not limited to, patient admission, patient rounds, generation of progress notes and patient orders, and patient discharge. NP activities will include staff development, research, and consultation. The NP acts as a consultant to the patient/family, nursing staff, institution, and community in the area of pediatric chronic and acute care needs. The NP interrelates with all departments in the hospital as a voice for nursing where clinical expertise is required. The NP collaborates with the Pulmonary and Gastroenterology physicians using a Standard of Care Arrangement.

The NP serves as an expert clinical nurse practitioner who utilizes the Synergy Professional Practice Model, specialized knowledge, skill, research, and competency acquired through advanced professional training to promote optimum care of patients and families within the pediatric population. The NP acts as a consultant to the patient/family, nursing staff, other employees and physicians and the community in the area of pediatric health. The NP collaborates with nursing leaders in developing annual goals as they relate to the hospital mission statement.
