Job Summary: The Advanced Practice Provider (APP) will provide diagnostic and therapeutic health care to patients through assessment, diagnosis, and treatment in collaboration with a designated PHMG physician. This role involves managing patient care from birth to geriatric age, focusing on acute illnesses within the ambulatory care setting. Responsibilities: Conduct patient assessments, diagnoses, and treatments for individuals from birth to geriatric age. Obtain and document comprehensive medical histories according to PHMG standards, state regulations, and collaborative agreements. Interpret and evaluate patient care data to determine appropriate management and treatment plans. Order and interpret diagnostic studies relevant to clinical specialty and perform minor and routine procedures. Encourage patient participation in their care through counseling, education, and discussion of treatment options and outcomes. Coordinate referral services and manage a variety of ancillary services. Prescribe, recommend, and regulate pharmacologic and non-pharmacologic therapies, including controlled substances as warranted. Consult with supervising physician as necessary.
