West Central Alabama AHEC Veterans Community Health Worker (Coordinator) in Demopolis, Alabama

Par-time position, 10-15 hours per week!

Job Duties

  • Serve as a trusted community liaison between participants, their families, health providers, VA facilities, and other community resources.

  • Assist participants in navigating health and human services systems and scheduling appointments.

  • Connect Veterans and their families to local veteran services officers for VA benefits enrollment and VA Health facility services.

  • Complete all required CHW training in a timely manner.

  • Coordinate and facilitate community events targeting Veterans and their families.

  • Identify, access, and accompany Veterans to community resources, supports, and services as needed.

  • Distribute surveys, collect data, and provide follow-up with participants via phone calls and home visits.

  • Conduct outreach activities, education, and baseline mental health screenings within the community

  • Utilize social media and other platforms to promote the program.

Qualifications

  • Military experience preferred and/or experience working with Veterans

  • 1 to 2 years of experience in a community based setting/health/social services preferred.

  • Basic computer skills required, including proficiency in Microsoft Office.

  • Strong organizational skills and experience with data collection/analysis.

  • Deep understanding of and connection to the community served.

  • Effective communication skills, including active listening and appropriate language use.

  • Ability and willingness to provide emotional support, encouragement, and motivation to participants.
