Griffin Health Lung Cancer Navigator Imaging Assistant in Derby, Connecticut

Description

MAIN FUNCTION: Provides administrative, technical, and clinical support for the Low Dose CT (LDCT) program; Provides clinical and administrative support for LDCT navigation office activities; Validates intake information and coordinates appointments for the Lung Screening Program patients; Processes necessary documentation for a LDCT. Provides point of service patient navigation including Lung View patient assessments, Smoking Cessation Enrollment, and normal test resulting for all enrolled patients. Collects, complies, analyzes, and reports relevant LDCT outcome metrics. Works under the direction of the LDCT Diagnostic Navigator to facilitate timely LDCT appointments and recommended follow up.

ESSENTIAL RESPONSIBILITIES & DUTIES:

  • Collaborates with the program LDCT Diagnostic navigator to begin eligibility process, break down barriers and open access to care.

  • Readily accepts assignments responsibilities related to the LDCT program to help ensure that patients are timely enrolled and have access to LDCT.

  • Participates in the Lung Cancer Leadership meetings with the LDCT Diagnostic Navigator as well as other clinical staff to help improve the program’s effectiveness. Coordinates meetings for departmental activities as directed by Department Director.

  • Coordinates with Central Scheduling for appointment availability, verifies insurance authorization for procedure and schedules patients accordingly.

  • Develops, creates and maintains clinical documents related to the LDCT program. Is able to navigate in LungView and maintain proficiency to work within the application as well as produce outcome data related to the LDCT program patients.

  • Demonstrates a high level of confidentiality. Is highly motivated and is a self-starter. Communicates with clinical team members well. Communicates appointment information to the patient, as applicable, providers and Lung Cancer Screening Diagnostic Navigator as needed.

  • Provides clinical care coordination services for the Lung Cancer Screening program,

  • Consistently coordinates lay navigation level patient services in a timely manner. Obtains physician orders as needed for additional imaging and ensures coordination of care. Works in LungView, Meditech Athena and EPIC as indicated to help coordinate care.

  • Responsible for providing information on and referring patients into the smoking cessation program.

  • With the Diagnostic Navigator, accurately tracks and timely reports outcome navigation, clinical and technical metrics for the Program. monthly via dashboard or report card.

  • Provides support for the program so the program can meet or exceed the American Radiology Lung Screening Designation requirements.

EDUCATION: AS in healthcare related field or equivalent preferred; ACS LION Oncology Navigation Certification within 6 months of hire. 1-3 years of healthcare experience and advance knowledge of medical terminology.

EXPERIENCE: Computer proficiency required, including advanced knowledge of Microsoft Office software, including Word, Excel, PowerPoint, Outlook. Competency in working with EPIC, Athena, LungView, and other applications. Oncology and/or diagnostic imaging experience highly preferred.
