Henry Ford Health System Program Manager Clinical & Social Health - CIN - Full time - Detroit in Detroit, Michigan
GENERAL SUMMARY:
Under the direction of the Director, Clinical and Social Health Integration (CSHI) for the Clinically Integrated Network (CIN), the Program Manager CSHI will execute specific stages of multiple CSHI programs across the Henry Ford Health System enterprise. The Program Manager, in collaboration with one or more departments/divisions, will have project management responsibilities over major CSHI program area(s), the goal of which is to support Henry Ford clinical and program team members to screen, assess, address, and monitor patient social determinant of health challenges.
CSHI program areas include food insecurity, transportation insecurity, housing instability, population-health focused primary care redesign, empathic and equity-driven social health care, and co-development of an external community-based social health information exchange. CSHI program areas have individual projects that are externally and internally funded to achieve CSHI aims. The Program Manager will play a key role in the management and implementation of departmental and/or system initiatives and strategies across CSHI program areas.
PRINCIPAL DUTIES AND RESPONSIBILITIES:
· Develop, execute, monitor, and track project management plans and outcomes to ensure timely completion of deliverables of various project workplans under assigned programs, seeking out guidance under limited supervision.
· Organize, convene, and facilitate meetings and relevant activities with project stakeholders.
· Maintaining regular communication with all parties on project progress and milestones; ensure all project team members have the resources and materials they need for completion of project activities.
· Conduct literature reviews and identify evidence-based best practices related to project activities and overarching program goals and objectives.
· Prepare technical, organizational, and project outcome reports for a variety of purposes, including internal monitoring, process improvement, outcomes synthesis, findings dissemination, and results publication.
· Monitor compliance with, and outcomes related to, various relevant Population Health value-based contracts related to the specific projects being managed.
· Support process evaluation and quality improvement activities, in partnership with collaborating departments, including drafting and coordinating Plan-DoStudy-Act (PDSA) or similar process improvement tests-of-change, to identify opportunities to strengthen project delivery and ensure projects are kept ontrack.
· Support outcome evaluation activities by monitoring short- and long-term project goals. Enter, collect, organize, and manage data, conduct basic statistical analysis and report on baseline statistics.
· Prepare reports, presentations, tables, graphs, utilizing data processing software and applicable tools, and present these communications verbally, in writing, and across a variety of platforms (in-person, virtual, conference, poster, etc).
· Access and incorporate available resources, tools, and/or materials (IT, analytic, program, and evidence-based/best practice) to support project management activities and clearly articulate and communicate areas of concern, needed tools, or barriers to practice achievement of goals to department leadership and other project team members.
· Provide direct management over project budgets, including ongoing monitoring of grant-funded budget spending, forecasting expenditures, accounting reports, completing relevant blanket purchase requisitions, processing invoices, and other financial requirements as necessary.
· Assist the Director, CSHI, in managing relationships with philanthropic funders and research entities for pilot and grant-funded projects, including preparing for meetings with relevant research and project findings, preparing timely project reports, and ideating future program and project activities for grant funding.
· Initiate collaborations and partnerships with other departments/divisions and with external stakeholders to ensure successful completion of the project.
· Ensure project sustainability in partnership with department leadership by suggesting improvement processes, monitoring business plan ROI, ensuring fidelity to project sustainability plan after pilot/grant-funding is completed.
· Establish successful working relationships and interact effectively with colleagues, leadership, committee and board members, providers, practice managers, community partners, management, and other staff to solicit input and buy-in from project stakeholders/end users and staff.
· Participate on various department and organizational project teams as assigned.
EDUCATION/EXPERIENCE REQUIRED:
· Bachelor’s degree in a healthcare or business-related related field (e.g. public health, health education, health administration, management, business administration, or related business/healthcare field). Master’s degree preferred.
· A minimum of 1-2 years’ experience in the design, coordination, management, and/or leadership of health programs. Certification, or interest in pursuing certification, in Lean Six Sigma, as a Project Management Professional (PMP), or similar is preferred.
· Demonstrated ability to work effectively in a complex environment and to negotiate and mediate complex and often competing interests.
· Demonstrated effective verbal and written communications, and interpersonal skills necessary to interact and build strong relationships with physicians, management, staff, external agencies and other key stakeholders.
· Proficient computer skills in word processing and spreadsheet software.
· Ability to interpret performance data from multiple sources to determine and implement effective action and solutions.
· Must meet or exceed core customer service responsibilities, standards and behaviors as outlined in the HFHS’ Customer Service Policy
· Must practice the customer skills as provided through on-going training and in-services.
PHYSICAL DEMANDS/WORKING CONDITIONS:
Normal office environment with minimal exposure to noise, dust, or extreme temperatures.
