Catholic Health Initiatives Coordinator RN in Devils Lake, North Dakota

Overview

Work in the heart of #humankindess at CHI St. Alexius Health Devils Lake.

As our Coordinator RN, you'll direct patient care in accordance with established policies and procedures of the nursing department, professional standards of nursing, applicable Federal and State laws, state nursing practice guidelines and regulatory agencies. Our Coordinator RN works collaboratively with all departments to assure highest quality of care for patients ranging from neonatal to geriatric.

You'll assure safe, efficient operation of the clinical department(s) to ensure a smooth operation which includes performance improvement activities, quality care, proper documentation to ensure compliance and completeness. This position will also monitor productivity and financial measures within the unit operations. Your attention to detail will be essential in developing, reviewing and revising policies and procedures in conjunctions with the nursing practice council. Your organization skills will ensure all necessary tools and supplies are available and staffing needs are met.

Furthermore, you'll utilize your leadership abilities as you will participate as a full, collaborative member of the nursing leadership council with other coordinators by participating in staffing, scheduling and budgeting. You'll mentor new staff directly, model positive and professional behavior and accountability.

To thrive in this role, you'll be an effective and clear communicator with physicians, co-workers and other departments to provide consistent care based on the changing physical, medical and psych-social needs unique to each patient. Provides feedback in a timely manner.

About Us:

CHI St. Alexius Health Devils Lake Hospital is a 25-bed Critical Access Hospital accredited by The Joint Commission licensed by the North Dakota State Department of Health and certified by the Department of Health and Human Services for participation in the Medicare Program. Our hospital has been committed to providing patients quality medical treatment in the Lake Region area and surrounding communities since 1902.

Responsibilities

  • Daily Nursing and CNA Assignments- works in collaboration with department supervisor to ensure safe staffing levels (twice daily staffing review with department supervisors) accounting for scheduled outpatient procedures outside of the SDS department

  • Collaborates with department supervisors for daily safety huddles

  • Daily Multidisciplinary Huddle

  • Attends safety first huddles daily

  • Rounding in all clinical departments

  • Patient Placement and admission coordination

  • Swing intakes (In conjunction with swing bed coordinator)

  • ER Admission

  • Surgery Admissions

  • Direct admits from the community

  • Works in collaboration with Case Management and primary nurse to coordinate efficient and complete patient discharge

  • Accountability to ensure Patient Experience tactics are in place - bedside report, intentional hourly rounding, leader rounding in collaboration with nursing leadership and hospital leadership team

  • Assures standards of patient care are met through quality measures

  • Infection Prevention - line and drain necessity

  • Fall Prevention - Environment and Documentation are appropriate and in place

  • Swing bed quality oversight

  • Improvement in self care

  • Improvement in Mobility

  • Serves as knowledgeable and experienced resource for staff

  • Works in collaboration with medical providers/hospitalists to ensure the safety and quality of patient care.

  • Complete initial assessments on admissions

  • Initiate care plans for all admissions

  • OB Postpartum or accepts patients to MSP if census allows.

  • Resource for Surgery/Same Day staff

  • Assist with patient care as needed

  • Assist with receiving outpatient orders if the SDS staff is tied up with patient care or pre-op calls - creating the account, confirming order with pharmacy, contacting patient, etc

  • Assist on days with higher case load

  • Assist as requested by department supervisor

  • Complete crash cart checks

  • Assures crash carts checks as outlined in hospital policy. If contents are missing/malfunctioning, it will be the responsibility of the Charge nurse/patient coordinator to replace/refill missing items.

  • Assures monthly outdates are completed and documented.

  • Assures glucometer quality controls are completed each night shift

  • Update midnight report and complete coordinator shift report

Social Determinant of Health

  • Assures social determinants of health assessments are completed on all patients

  • Communicates with community health advocate to assures information on available services are provided to patients

  • Documentation of education with patients on available resources for identified needs

Qualifications

  • Graduate of an approved school of professional nursing.

  • Associate of Science in Nursing.

  • Active Registered Nurse (RN) License in the State of North Dakota (ND).

  • American Heart Association (AHA) Basic Life Support (BLS).

  • ACLS, PALS, NRP, TNCC

  • Ability to operate computer programs such as Google Suite, Meditech, Med Assets, and Milliman.

Pay Range

$33.60 - $48.73 /hour
