Position Summary

Under the supervision of and reporting directly to the SVP of Nursing and Clinical Operations, the Clinical Based Community Services Coordinator will be responsible for assisting with project implementation of various clinical programs and community events, including vaccination events, both public and specific to special populations and/or school districts. May be instrumental in coordinating with other organizations including but not limited to state agencies to schedule special events such as retinal screening events or to oversee Zufall’s participation in statewide initiatives such as Monkey Pox testing. Specific responsibilities include assisting with project planning, organization, delivery, and data gathering for reporting to grantors and funders of these programs; meeting with implementation team and staff and outreach team as needed; developing program deliverables after consultation and collaboration with supervisor and the teams; and training staff on workflow processes, grant or project content. The Clinical Based Community Services Coordinator will meet with the supervisor, site managers and staff that are directly providing the services to communicate deliverables and program goals, to help develop a project implementation plan, to determine needed training and workflow changes, to follow up on project requirements and to assist with providing program guidance, materials, and direct services. At times, will be required to interact with the community, especially during screening or group events.

Essential Functions, Duties, and Responsibilities

  • Has overall responsibility and oversight of assigned programs

  • Reviews and understands project designs, deliverables and plans to implement and reach stated goals

  • Maintains confidentiality of all patient health information and proprietary Zufall

  • Translates project goals and objectives into action plans, by meeting with the project teams and by getting input from the team to develop best practices

  • Develops and completes reports and project deliverables

  • In collaboration with the QA Program Manager and Data Analysts, assists in the development of workflow to generate data and produce reports that are useful for the program staff, the team, and funders

  • Participates in meetings, collaboratives and training sessions and other meetings as required by the agency and/or the funding sources, as needed

  • Attends and contributes to meetings with the different teams and supervisors and identifies and/or develops resources and patient education materials

  • Responds to issues effectively and in a timely manner

  • Attends and participates in appropriate staff meetings and QA/PI meetings, task forces and subcommittees, and travels to different sites as needed. Participates in virtual meetings and conferences

  • Prepares end of month reporting and/or participates in program evaluation as requested

  • Participates in program activities and community outreach events

  • Demonstrates appropriate behavioral boundaries including treating persons with respect, and abides by the code of conduct expected of Zufall employees

  • Collaborates with clinic staff to ensure quality care and identify appropriate services

  • Fulfill other duties as assigned by the SVP of Nursing and Clinical Operations or their designee

    Knowledge, Skills and Abilities

  • Excellent oral and written communication and computer skills

  • Strong communication skills, problem solving, teamwork and organizational skills

  • Knowledge of logic model and action plan development

  • Ability to work cooperatively as a part of a team in a clinical and non-clinical setting

  • Ability to work with multicultural and diverse population. Bilingual (Spanish)/bicultural preferred

  • Must be self-motivated, detail oriented, able to travel locally and work flexible hours

  • Learn and apply trauma informed care principles with the scope of the position

    Education, Training and Experience

  • Bachelor’s Degree in a related field of study

  • Three years of clinical community-based work

  • Experience working with underserved populations

  • Experience running programs and organizing teams

  • Working knowledge of a Windows-based computer system, Microsoft Office programs and publishing software

    As a condition of employment, we require that all staff are fully vaccinated against flu.
