Job Summary: As a Housing Case Manager, you will provide case management services to individuals who are currently residing in permanent supportive housing. This facility-based position will support 10 youth, assisting with living independently, coordination of services and maintaining housing stability. You will work in collaboration with the Facility Coordinator to assess youth for eligibility, complete all necessary paperwork to secure housing, and provide ongoing assistance and support to maintain their housing. You will facilitate skills groups/individual skills work and collaborate/connect with other necessary service providers/community resources to assure youth needs are met. Documentation, case planning and crisis management are key responsibilities of this position. Qualifications and Requirements: Bachelor's degree in Social Service or related field, or 1 year working with youth or housing services and lived experience in lieu of a degree. A valid driver's license, acceptable driving record, reliable transportation, valid insurance, and a Motor Vehicle Check. Strong organizational skills. Ability to be focused and attentive for an extended period of time. Able to develop, guide, and provide follow-up for client needs. Ability to promote a positive attitude regarding LSS and other programs to the public. Knowledgeable regarding housing resources and community resources.
