Case Manager Job Description Job Overview: Founded in 2016, The Margaret Home is a residential program that revives hope for mothers who maintain a limited or broken support network and who struggle with the often-invisible wounds of trauma. Dedicated to serving women through all stages of pregnancy and early motherhood, we provide healing and unconditional love in a comfortable home and caring communal environment that nurtures an empowered spirit and a healthy lifestyle. The Margaret Home is hiring a Case Manager. Interested candidates should be experienced, motivated professionals who will provide case management, guidance, and advocacy for the mothers and children. Interested applicants may have a social work degree. Case Manager works with each mother to create a self-directed action plan designed to enable the achievement of personal, educational, and professional goals that can lead to independence for each mother and her family.

Requirements: Bachelors degree in social service field desired . Experience in Case Management or willingness to be trained Dedication and full support of the mission of The Margaret Home to promote the sanctity of life and the dignity of women

Primary Responsibilities: Completes the Intake process for mothers in collaboration with the Operations Manager Provides case management for mothers Conducts one-on-one case management sessions. Assists in formulation of individualized service plan with mothers. Promotes linkages with community organizations and businesses. Monitors the provision of services from community agencies to ensure appropriateness of care for mothers Works with Operations Manager on volunteer coaching staff. Responsible for follow-up case work on alumni mothers for a minimum of one year. Maintains information, database driven, on all mothers. Ensures that mothers make and are able to keep their outside appointments. Assists in the implementation of enrichment programs and special events.

All applicants should be mission-driven, flexible, and willing to self-motivate. Applicants should be willing and able to work closely with other team members and volunteers, and collaborate regularly with internal staff and board members, demonstrating an ability to lead and coordinate as work is delegated. Applicants will have some flexibiltiy in scheduling as well as an option to bring own baby to work with supervisor's approval.
