Description

The Advanced Practice Provider (Nurse Practitioner/Physician Assistant) functions as an integral health care provider to enhance the quality of patient care outcomes. Primary responsibility includes:

  • Performing pre-transplant health and physical evaluations of potential kidney transplant candidates; providing education to patients.
  • Routine follow-up and management of post-transplant patients in the hospital and post-transplant clinic.
  • Triage patient calls and provide medical advice/direction as necessary.
  • Prescription refills for post-transplant patients.
  • Management of kidney transplant patients during initial transplant admission as well as re-admissions.
  • Scheduling transplant biopsies, ultrasounds, IVIG, and plasmapheresis.
  • Review test results, lab reports, and consults to assist in patient management for the transplant waitlist.
Qualifications

REQUIRED QUALIFICATONS:

  • For a Nurse Practitioner:
    • Current New York State Registered Nurse License.
    • Master's Degree in Nursing (foreign equivalent or higher).
  • For a Physician Assistant:
    • Bachelor's Degree (foreign equivalent or higher).
    • Graduated from an ARC-PA accredited program.

PREFERRED QUALIFICATIONS:

  • Experience with end stage renal disease (ESRD).
  • Experience with pre- and post- operative care of kidney transplant patients.
  • Critical care experience.
  • Management of patients with co-morbidities.
  • Knowledge of PowerChart, Microsoft Word, Excel, Outlook.
