Job Summary:

Primary functions are to administer nursing care for hospice patients in their place of residence, coordinate care with the interdisciplinary team, patient/family, and referring agency; and assumes the responsibility for coordination of care.

Job Qualifications:

Graduate of an accredited school of Nursing either an Associates Degree or a Bachelor's Degree. All applicants must have a current Texas license as a Registered Nurse, and a Texas Drivers License, dependable transportation, Valid, andcurrent auto liability insurance. Two-year experience as a Registered Nurse in a clinical care setting. Good Interpersonal skills and a compassionate nature Collaborate with physicians and other professionals to carry out the plan of care for each patient Work with caregivers to plan and execute excellent patient care Maintain accurate records
