Job Description

The case manager will provide support and service linkages to residents within the program. Coordinate services and ensure quality care to the residents.

Essential Accountabilities:

  • Routinely visit apartments and moving freely through the apartments to observe and evaluate level of functioning of residents.

  • Provides direct care services to resident, including planning and implementation of goals and training, which will develop and enhance activities of daily living skills. These include but are not limited to:

  • budgeting

  • grooming

  • nutritional planning

  • use of public transportation/travel training

  • use of community resources/community integration

  • shopping/menu planning

  • housekeeping

  • socialization skills

  • physical health maintenance

  • assertiveness/self advocacy training

  • Responsible for all necessary written documentation including: functional assessments, initial and updated; medication update; progress notes and reports; annual and quarterly service plans; etc.

  • Screens candidates for residential placement this may require attendance at hospital or day treatment program; participates and attends meetings/trainings at various sites that require following different route of travel to worksite; participates in resident meetings and conduct all 30-day follow-ups on discharged residents.

  • Assists residents in planning and coordination of recreational/social activities, this may require escorting residents to these activities and/or participate in moderate physical activities as part of this task.

  • Attends all staff meetings; ensures that all resident meetings take place on a timely basis.

  • Accountable for the opening and all follow-ups necessary for new apartments; responsible for the maintenance, up keep and safety of assigned apartments.

    1. Reports and documents all incidents to immediate supervisor and/or the Director of Behavioral Health within 24 hours; maintains all resident files on a monthly basis.

    2. Responsible for crisis intervention.

    3. Monitors environmental plant issues and report all issues to immediate supervisor and/or Director of Behavioral Health.

    4. Participates in activities of other staff members in their absence or during periods of staff shortage.

    5. Represent the agency at meetings and trainings not otherwise specified

Job Qualifications

A n Associates Degree in Human services; or a High School diploma with two (2) years experience in the Human Services field. New York State drivers license and access to a vehicle required.

Demonstrate ability to: work flexible schedule as work schedule and locations are subject to change; communicate information with all staff and supervisor(s) verbally and in writing for the purpose of giving instructions, providing feedback and adjusting work practices; count, add, subtract, and manage money; monitor resident progress, i.e.: duties include, but are not limited to: seeing, hearing and observing residents, performing manual tasks, reading and writing (documentation, lifting, climbing and descending stairs, traveling, boarding and de-boarding agency vehicles) and utilize public transportation (bus, subway) use of telephone to make and receive calls; lift/carry packages and boxes of supplies; as well as clients if necessary; coordinate evacuation and ensure residents safety in emergency situations (i.e., fires, fire drills); exit in emergencies, which may require the ability to climb and descend fire escapes and assist others to do the same; complete the following training offered and perform the techniques taught in each area a required function of the position; Case Management- 101, CPR and First Aid, Cultural Competency, CPI, tip42, Motivational Interviewing, Suicide Assessment and Prevention, drive facility vehicle when the need arises (escort, field trips, etc.); transport self to various sites.
