UPMC Service Coordinator Lead (Safe Harbor Behavioral Health) in Erie, Pennsylvania

UPMC Western Behavioral Health at Safe Harbor is looking for a passionate Service Coordinator Lead to support a new initiative dedicating case management service to individuals in our community through an approach that prioritizes permanent housing for people experiencing homelessness by providing basic necessities so they may pursue personal goals.

At Safe Harbor, we understand that housing is healthcare, and it takes a personal and trauma-informed approach to keep our neighbors in our community in stable housing. To address the increasing complex needs around housing instability the Service Coordinator Lead will operate under the model of Cultivating Health for Success (CHFS). The CHFS model provides specialized case management to identify and address barriers to create individualized care plans based on an assessment addressing the social determinants of health.

As a Service Coordinator Lead , you will be active within the community, working directly with members of Erie County, providing case management services to address instability, with the focus on preparing clients to become housing ready. Schedule & Location:

  • Work Hours : 8:30a-5pm Monday through Friday with some occasional evening and weekend hours. Additional 24/7/365 on call shifts required.

  • Location : 1330 West 26 th Street (Home office location)

  • Parking : Provided at home office at no cost

  • Travel : Reliable vehicle & driver’s license required. Milage reimbursement available.

Responsibilities:

  • Program Operations:

  • Follow Service Coordination guidelines, support daily operations, and assist with recruitment/training of team.

  • Staff Management:

  • Evaluate, supervise, and provide feedback to Service Coordinators.

  • Compliance & Quality:

  • Conducts audits, ensure regulatory compliance & handle complaints/investigations.

  • Community & Training:

  • Attend trainings, identify needs & maintain community connections.

  • A master's degree in social work, psychology, rehabilitation, activity therapies, counseling, or education AND 3 years of mental health direct care experience

  • OR a bachelor's degree in sociology, social work, psychology, gerontology, anthropology, history, criminal justice, theology, counseling, education, AND 3 years mental health direct care experience

  • OR a bachelor's degree in nursing AND 3 years mental health direct care experience. Licensure, Certifications, and Clearances:

  • Cardiopulmonary Resuscitation (CPR)

  • Comprehensive Crisis Management (CCMC)

  • Act 31 Child Abuse Reporting with renewal

  • Act 33 with renewal

  • Act 34 with renewal

  • Act 73 FBI Clearance with renewal

  • Driver License
