UPMC Service Coordinator Supervisor (Safe Harbor Behavioral Health) in Erie, Pennsylvania

UPMC Western Behavioral Health at Safe Harbor is seeking a passionate Service Coordinator Supervisor to lead a new initiative focused on providing case management services individuals in our community. This approach prioritizes permanent housing for people experiencing homelessness ensuring they have basic necessities to pursue their personal goals.

At Safe Harbor, we understand that housing is healthcare, and it requires a personal, trauma-informed approach to maintain stable housing in our community. To address the growing complexity of housing instability, the Service Coordinator Supervisor will follow the Cultivating Health for Success (CHFS) model. This model offers specialized case management to identify and address barriers, creating individualized care plans based on assessment of the social determinants of health.

As a Service Coordinator Supervisor , you will be active within the community, working directly with members of Erie County, providing case management services to address instability, with the focus on preparing clients to become housing ready.

You will operate as a Housing Specialist driving the daily program operations through:

Team Leadership:

  • Hire, supervise, support & provide clinical guidance to the service coordination team and peer specialists

  • Promote diversity, equity, inclusion, wellness trauma-informed care & professional development

Strategy & Program Development:

  • Develop strategies with partners & participate in program meetings

  • Implement & refine case management training, policies & procedures

Data & Quality Assurance:

  • Manage data outcomes & reporting

  • Audit daily operations for quality assurance

Partnerships & Collaboration:

  • Build & maintain partnerships to meet client needs

  • Meet weekly with property managers to monitor service delivery & housing operations

Schedule & Location:

Work Hours : 8:30a-5pm Monday-Friday, with some occasional evening and weekend hours. Additional 24/7/365 on call shifts required.

Location : 1330 West 26 th Street (Home office location)

Parking : Provided at home office location at no cost

Travel : Reliable vehicle & driver’s license required. Milage reimbursement available.

Responsibilities:

  • Follow competencies & perform duties.

  • Participate in weekly on-call rotation.

  • Assist with service advocacy.

  • Provide & attend training.

  • Develop policies and procedures.

  • Supervise up to nine coordinators.

  • Manage program operations & compliance.

  • Oversee caseloads & performance.

  • Monitor on-call rotation.

  • Assist with service plan appeals.

  • Collaborate on case reviews.

  • A master's degree in social work, psychology, rehabilitation, activity therapies, counseling, or education AND 3 years of mental health direct care experience.

  • OR a bachelor's degree in sociology, social work, psychology, gerontology, anthropology, history, criminal justice, theology, counseling, education, AND 5 years mental health direct care experience, 2 of which shall include supervisory experience.

  • OR a bachelor's degree in nursing AND 3 years mental health direct care experience. Licensure, Certifications, and Clearances:

  • Cardiopulmonary Resuscitation (CPR)

  • Comprehensive Crisis Management (CCMC)

  • Act 31 Child Abuse Reporting with renewal

  • Act 33 with renewal

  • Act 34 with renewal

  • Act 73 FBI Clearance with renewal
