The Case Manager will be responsible for a variety of duties, which could include responding to contacts seeking information regarding services and supports for individuals with cognitive disabilities, assembling information about the individual for eligibility determination, providing Case Management services until the person can be admitted into services, being responsible for the assessment planning, coordination, and monitoring of services and supports provided to individuals with intellectual/developmental disabilities and their families as appropriate.

Requirements: Bachelor's degree required; or five years of relevant experience in the field of Long-Term Services & Supports, which includes Developmental Disabiliti Driving is a requirement for this position. Prior case management and/or advocacy experience strongly preferred. Must be comfortable working with and near individuals with intellectual/developmental disabilities. Must display a high degree of responsibility, initiative, and interpersonal and communicative skills.
