SUMMARY:

An immediate opening for a Triage Nurse (RN or LPN) supporting their Fort Wayne clinical team within a leading organization for kidney health and research! They provide top tier physician care to patients facing kidney disease, supporting treatment plans for best quality of life. They have over 170 physicians and nurse practitioners supporting thousands of patients. Their demand continues to increase, and so is their staff at all sites, especially the Fort Wayne location.

We are seeking a Triage Nurse that has practiced in specialty physicians groups providing quality care and support to patients and providers to apply that expertise to support the nephrology patients and physicians. This is a very collaborative, fast pace office based role that is day shift Monday-Friday with no weekend or holiday work.

DAILY RESPONSIBILITES:

  • Support 6-7 physicians daily with incoming clinical calls and messages, which would be speaking to 30-40 patients per day (call times range)

  • Update patient health information systems with notes and observations. Using quality documentation, computer and system navigating, and time management skills

  • Field calls as needed for best treatment support

  • Answer comprehensive clinical questions for patients regarding their treatment plans and physician orders

  • Conducting initial assessments to evaluate the urgency of a patient's condition.

  • Communicating with physicians, nurses, and other healthcare professionals about patients' statuses

  • Build trust and rapport with patients and help them navigate the nephrology process

  • Educating patients and their families about their conditions and the next steps in their care

REQUIRED QUALIFICATIONS:

  • RN or LPN. State or Compact Nursing License Required

  • 2+ Years of nursing experience in a specialty unit or physician office

  • Positive Attitude, Self-Motivated, Team Player, Strong Communication, Data Documentation, Time Management, and Organization

PREFERRED but NOT Required:

  • Any experience within triaging and clinical experience in Nephrology, Kidney Dialysis, Cardiology is a plus

  • Triage experience preferred
