Advocates Registered Nurse - Shared Living in Framingham, Massachusetts

Overview

Hourly Rate: $ 46.50

The Shared Living Registered Nurse is responsible for the coordination high quality medical supports to the individuals and provider families served by Shared Living program (SHL). The Shared Living RN provides case management and medical teaching and training to families and participants.

Responsibilities

  • Liaison with other agencies involved in providing services to the individuals we support; such as day programs.

  • Ensure that the program meets and complies with all applicable Adult Foster Care Medicaid regulations: oversee compliance of families with mandated trainings.

  • Review and assess medical needs of new referrals, as well as new families.

  • Complete all nursing assessments; MDS-HC and request for services, for each referral packet.

  • Collaborate with the Shared Living Coordinator to ensure completion and availability of all medical documents needed for ISP meetings.

  • Participate in recruitment, selection, and evaluation of families. Interview families and facilitate the matching and transition of individuals into homes.

  • Develop positive relationships with individuals and shared living families; monitor and assess placements in collaboration with the Shared Living Coordinator.

  • Train and supervise participating families on all aspects of their responsibilities, including the participant’s medical and personal care needs, as well as their overall health status.

  • Train and assist families on an on going basis of all aspects of medical health concerns pertaining to the individual, including advocating with physicians and other health care providers.

  • Monitor the individual’s health status on an on going basis through home visits and regular contact with the family.

  • Maintain accurate medical records, documentation and reviews; to include, daily log, vital signs, medication sheet, monthly nursing notes, and health records. Develop and complete participant’s semi-annual health status report, Plan of Care, and annual Minimum Data Set assessment.

  • Support the medical development and growth of all individuals served in the program.

  • Develop positive relationships with the clinical nursing staff within all State agencies.

  • Provide crisis intervention and 24 hour on call responsibility.

  • Oversee grievance policy for families and individuals.

  • Plan and implement discharges from the SHL program

  • Participate in the state wide MA council for AFC monthly meetings.

  • Attend all trainings as assigned.

  • Attend and actively participate in supervision and team meetings.

  • Ensure that individuals we support are treated with dignity and respect in accordance with Advocates’ Human Rights Policy.

  • Perform all duties in accordance with the agency’s policies and procedures.

Qualifications

  • B.S in nursing and must possess a current, state of MA, registration in nursing and one year of experience in a community health care setting.

  • Must have strong oral and written communication skills with consumers, families, funding sources, consultants, the community at large, and the program and administrative staff.

  • Must be able to perform each essential duty satisfactorily.

  • Must have sensitivity to the needs of the population.

  • Must have basic computer knowledge.

  • High energy level, superior interpersonal skills and ability to function in a team atmosphere

  • Must hold a valid drivers’ license. Must have access to an operational and insured vehicle and be willing to use it to transport consumers.
