Trinity Health Respiratory Navigator Full Time in Ft. Lauderdale, Florida

Description:

This position will be located in the new state of the art COPD Clinic, and a part of our grand opening of our new comprehensive pulmonary rehab program.

The role of the pulmonary rehab respiratory navigator in providing a range of care and therapy interventions for people who have Chronic Obstructive Pulmonary Disease (COPD), Bronchiectasis and other chronic lung conditions. Develop effective, engaging, and motivating educational topics enabling the patient to best self-manage chronic lung disease.

Use judgement and initiative to produce exercise programs, develop them and see them through to completion

On-going data input which enables statistics to be collated from this Information.

Basic understanding of pulmonary disease management and respiratory care services including home health durable medical equipment

Able to work with patients to build relationships and develop methods for self -care

Pulmonary function testing

ESSENTIAL FUNCTIONS A N D RE SP O NS I B ILI T IE S :

Serve to promote best practice in pulmonary patient care and assimilate various service lines to meet patient needs

Abili t y to d e velop po s iti v e, longi t udin a l relationshi p s and s et ap p r o priate b ou ndaries wi t h patie n t s /familie s .

Provide planned interventions and therapeutic rehabilitation program with adults suffering from COPD, Bronchiectasis and other chronic lung conditions.

Provide administrative support for running the pulmonary rehabilitation courses. This includes organizing assessments, education and liaising with patients

Minimum qualifications

Registered Respiratory Therapist (RRT) from the Department of Health.

BLS/ACLS

Knowledge in Pulmonary Function Testing

Pulmonary Disease educator certified or willing within 6 months

Pulmonary Rehab certification

2+ years experience in pulmonary chronic care
