Glens Falls Hospital Registered Nurse (RN) - Case Manager (Emergency Services) in Glens Falls, New York

The Impact You Can Make The Case Manager (Emergency Services) (CM ED) reviews patients from all points of entry for care management needs (emergency room/direct admits, transfers, elective procedure schedules, etc.). The CM ED uses case management knowledge to promote appropriate allocation of hospital and other resources, while striving to provide high quality care to each patient in a cost-effective and timely manner. Support services provided by the CM ED include, but are not limited to, utilization review, case management, physician collaboration for status and level of care recommendations, care transition/coordination, and discharge planning. In addition,the CM ED offers assistance and intervention to patients and families with complex psychosocial needs (refers to Social Worker as needed) and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. The CM ED works in collaborative practice with Physicians and Physician extenders to actively support the delivery of quality, cost effective, outcome oriented, healthcare in the appropriate care setting to meet patient specific, age, and cultural needs. The Glens Falls Hospital Impact Mission Our Mission is to improve the health of people in our region by providing access to exceptional, affordable, and patient-centered care every day and in every setting. * * How You Will Fulfill Your Potential Responsibilities * Maintains/enhances clinical expertise and utilizes this knowledge to ensure patient focused quality care across the health care continuum. * Accesses all appropriate data sources to initiate assessment process (including test results, medical record documentation and physical and psychosocial assessment of patient) to illustrate patient acuity. * Performs the initial review on all admissions using the appropriate InterQual guidelines or in accordance with CMS rules and regulations for admission and medical necessity. * Ensures order in medical record coincides with the status based on InterQual review or CMS rules and regulations for appropriate Level of Care and status on all patients. Collaborates with appropriate provider and nursing staff when order does not coincide. Education & Experience * Bachelors of Nursing (BSN) is preferred at hire, and is required within 5 years of hire. Exceptions may be made for Associate’s degree RN with relevant experience and obtains certification in case management (CCM or ACM) within 1 year. * Minimum of two years of experience in a hospital environment strongly preferred * Home care, payer, or other experience will be considered Licenses, Certifications, Registrations * Registered Nurse (RN) in New York State Skills and Abilities * Ability to function autonomously maintaining a high level of clinical and professional accountability * Demonstrates skill in creative problem solving, facilitation, collaboration, coordination and critical thinking * Embraces change and continuously identifies opportunities for improvement by demonstrating a commitment to creativity and innovation * Committed to promoting excellence in Customer Service; functions as a team player * Computer literacy and data analysis skills are required * Maintains professional image by demonstrating strong verbal and written communication skills * Demonstrates ability in self-starting, self-directing and clear decision making behaviors Communities We Serve Located in the foothills of the beautiful Adirondack mountains, Glens Falls is conveniently located a short drive away from the capital region and Lake George. Salary Range The expected base rate for this Glens Falls, New York, United States-based position is $74,000 to $106,252 annually. Exact rate is determined on a case-by-case basis commensurate with experience level, as well as education and certifications pertaining to each position which may be above the listed job requirements.
