The Impact You Can Make The Case Manager (Emergency Services) (CM ED) reviews patients from all points of entry for care management needs (emergency room/direct admits, transfers, elective procedure schedules, etc.). The CM ED uses case management knowledge to promote appropriate allocation of hospital and other resources, while striving to provide high quality care to each patient in a cost-effective and timely manner. Support services provided by the CM ED include, but are not limited to, utilization review, case management, physician collaboration for status and level of care recommendations, care transition/coordination, and discharge planning. In addition,the CM ED offers assistance and intervention to patients and families with complex psychosocial needs (refers to Social Worker as needed) and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations. The CM ED works in collaborative practice with Physicians and Physician extenders to actively support the delivery of quality, cost effective, outcome oriented, healthcare in the appropriate care setting to meet patient specific, age, and cultural needs. Collaborates with appropriate provider and nursing staff when order does not coincide. Education & Experience * Bachelors of Nursing (BSN) is preferred at hire, and is required within 5 years of hire.
