Position Purpose:

Our LPNs provide skilled nursing care to the organization’s clients in accordance with the plan of care as prescribed by the physician and assigned by the supervising registered nurse. These nursing activities are limited to those in compliance with the state’s/province’s LP(V)N/RNA Practice Act, any applicable licensure/certification requirement, and agency policies and procedures.

What You Will Do:

  • Effectively and efficiently plans visit content and schedule based on client capabilities, condition, preferences (when possible), physician orders and available resources.

  • Performs specific treatments and administers medications to implement the medical and nursing care plans.

  • Apply appropriate preventive, therapeutic, and rehabilitative nursing procedures, and techniques to clients.

  • Instructs family members in client care.

  • Assists in care plan revisions as indicated.

  • Participates in care conferences with other members of the health care team.

  • Utilizes and implements appropriate tools and pathways per guidelines and/or policy and procedure (e.g. education checklists, pathway protocols, Home Program Instructions, etc.).

Minimum qualifications:

  • Graduate of an accredited or state approved school of practical nursing

  • Licensure as a LPN in the State of Michigan

  • A minimum of five (5) years of experience in an acute care setting; home care experience preferred.

  • Phlebotomy and wound care experience required.

  • Must have passed National Association Practical Nurse Exam (NAPNES) or equivalent medication exam before administering medications.
