About the Work Unit:

This work unit is to provide services and support, quality care and training to individuals in the Day Program at the Grand Junction Regional Center (GJRC). Services and support are to assure that individuals receive education, training, and therapy needed to assist them in achieving their goals, and to provide opportunity for choices during work, leisure, and community activities. The unit facilitates outcomes for people with developmental disabilities or who have a dual diagnosis in a variety of program settings that together achieves a therapeutic milieu for up to 15 persons.

Description of Job

About the Position:

This position exists to ensure that personal outcomes are achieved for people receiving services through the management of one or more day program settings, and community or work activities as described in their Individual Plan. Duties in this position include:

Provide Day Programming for Individuals Receiving Services:

  • provide direct hands-on programming to individuals assigned to the Day program area

  • manages and is responsible for the operation of a primary work area or job site

  • organizes work or program area, obtains equipment and supplies

  • acts as coordinator for residential staff and up to 15 people in a day program or work setting

  • acts as a resource or advisor to others, orient new staff to the work area and to individual’s programs

  • refers issues, concerns, or recommendations about performance to the appropriate supervisor, may take the lead role when supervisor is absent to include scheduling the day’s events and program coverage

  • motivates staff working alongside them in the day program area to assist the individuals served in achieving the highest level of achievement

  • implementation and the understanding of the need for daily use of electronic health records for documentation

Identification of Training Needs:

  • identifies training needs for staff and individuals receiving services as it relates to achieving outcomes in their work area

  • refers training needs to therapists or supervisors, coordinates, schedules, and conducts training to meet these needs

  • annual recertification is required for “CORE” skills including CPR, suctioning, transfers and lift use, wheelchair securement, Safety Care, additional mandatory training is presented through Continuing Education on an annual basis

Monitor Work Production and Lead Work:

  • monitors the production of others; communicates to assigned staff specific expectations and provides feedback

  • monitors individual’s wages per Department of Labor regulations

  • consistently act as a role model to assorted staff

  • provide accurate data of services provided and the individual’s responses to the program: Follow defined documentation guidelines for daily use of electronic health record for documentation for programs, progress notes, evaluations, attendance sheets, payroll sheets, etc

  • plans for and allocates resources to meet both annual and daily needs in their primary work area, this includes reconciliation of funds, staff scheduling, transportation, and other resources

Assist in Other Areas:

  • assists with achieving outcomes for people in related areas as needed or as directed. This may include assisting with residential, therapy, recreation, or other needs. This may occur outside normal work hours to include evenings and weekends

  • may be required to work other locations and/or shifts as needed to address agency resource needs

  • may be required to transport individuals on state vehicles from home to programming site or to offsite work areas, as well as taking individuals on outings

  • administer all treatments, medications, and dietary orders as prescribed

  • responds to emergency situations and assists medical staff with medical support and services, including preparing meals to prescribed diet consistencies as specified in the dietary planning menu. Variances to orders must be documented and as needed have responses as directed by Health Services staff

Ensure Compliance and Other Duties as Assigned:

  • ensures compliance by staff with Medical, Psychology, OT, Speech, Recreation, Habilitation, and PT needs and recommendations

  • assist therapists with specialized evaluations as required

  • routinely communicate with others about work/program assignments and expectations. Demonstrate the ability to explain program rationale

  • respond to behavioral challenges as needed with implementation of specific Behavior intervention programs (including Safety Control or Emergency Control)

  • must be annually certified for the skilled implementation of Safety Care, interventions are to be from the least invasive and restrictive and only progressing should safety to a person or peers be imminent

  • handle any issues which may arise to ensure smooth operation of the Day Program

Minimum Qualifications, Substitutions, Conditions of Employment & Appeal Rights

Minimum Qualifications:

  • high school diploma or General Education Diploma (GED)

  • two years of relevant experience in the related field

Your experience must be clearly documented in your application. Part-time experience will be prorated.

Substitution:

Additional appropriate education will substitute for the required experience on a year-for-year basis, but not for the license/certification requirements or computer proficiency requirements.

Conditions of Employment:

  • a pre-employment criminal background check will be conducted as part of the selection process as this position is deemed to have direct contact with vulnerable persons. Felony convictions, conviction of crimes of moral turpitude, or convictions of misdemeanors related to job duties may disqualify you from being considered for this position. Should your background check reveal any charges or convictions, it is your responsibility to provide the CDHS Background Unit with an official disposition of any charges. Additionally, you will be required take a drug screen.

  • shift Work – GJRC is a 24/7 facility. Primary shift if 1st shift

  • TB test is required, requires Influenza and vaccines as required by State and facility

  • drivers License - license must be valid and unrestricted and remain so throughout employment. Inability to drive a state vehicle, could result in a loss of employment.

  • travel - Drive state car, van or bussette to various group homes, programming, admin & in community

  • former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS
