POSITION OVERVIEW:

This position is responsible for coordinating the Therapeutic Navigation Services for Montrose, Delta, and Mesa counties, and operates out of the Montrose Family Resource Center with the High Fidelity Wraparound team. This includes hiring and supervising staff, training, program development, budgeting, quality and development, grant/contract objectives and progress monitoring and reporting. Carries an appropriate direct service caseloads

This position has a schedule of Monday - Thursday 8a-5p, Friday 8am-12pm, with weekends and nights as needed. The hiring wage range for this position is $22.50-$23.62 per hour.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Hire, onboard, train, support and supervise assigned staff. Coordinate and provide oversight to Therapeutic Navigation Services and ensure quality and fidelity of the program. Coordinate and provide oversight to therapeutic navigation services program. Track and enter data when appropriate. Oversee data collection systems and outcomes. Review monthly billing for therapeutic based services and submitting to program manager. Organize outreach and community education to maintain referral levels. Oversee and manage referral process to enroll individuals and families in therapeutic navigation services. Monitor case notes and advise staff on participant cases. Prepares, monitors, and adheres to monthly budget. Ensures revenue and expenses are properly tracked. Track and ensure compliance for credentialing of navigators providing therapeutic based navigation. Grant and contract management to ensure all requirements are met and adhered to. Participate in company trainings, attend community collaborative meetings, carries out duties associated with therapeutic services. Provide direct services and facilitation for an appropriate caseload; which may include home visitation, assessment, and case management. Advise and assist in program design, including the development and implementation of programing, contract, grants, and fee schedule, based on need, with the Program Manager.
