Corewell Health Community Health Worker in Grand Rapids, Michigan

Hours of Work: 8:30 a.m. to 4:30 p.m., variable for outreach

Job Summary

Start Now Program is a chronic disease management program. The goal of the program is to improve the quality of life and self-efficacy of persons diagnosed with chronic conditions through education/training, individualized goal setting, and resource navigation. Community Health Workers are an integral part of our health team model. Cultural competency and shared-life experiences with client population is preferred.

Essential Functions

  • Evaluates program participants’ educational needs, and social determinants of health when creating plans of care and employs appropriate teaching strategies while continually evaluating progress toward achievement of goals/outcomes.

  • Assists RN Care Managers with biometric screenings by helping weigh participants, check blood pressures, cholesterol, and blood sugar. (All clinical skills will be taught during orientation.)

  • Communicates and collaborates with other members of the health care team to ensure continuity and coordination of care.

  • Maintains knowledge of current trends and developments in the field by reading attending related conferences and maintaining CHW certification.

  • Participates in community events and activities that promote the awareness of health maintenance and disease prevention.

  • Assists with the development and implementation of care plans to meet identified needs. Follows up with clients, and community agencies to evaluate effectiveness of services provided and to develop a proactive plan for future needs.

  • Assists clients with arranging and keeping medical appointments. Assists clients in accessing community resources such as food, clothing, shelter, medical insurance, and other social determinants of health.

  • Communicates and collaborates with the clients and their families, program staff, and other professionals involved in the care of clients to coordinate services and to facilitate a supportive relationship between the client and their health care providers.

  • Documents services including assessments, plans, and periodic evaluations of goals, education, and other pertinent program data. Provides input into program development and evaluation.

  • Willing to visit participants in the community or home setting if necessary.

Qualifications

Required

  • Required High School Diploma or equivalent and successful completion of the CHW Training Program (Completion within 180 days from hire date)

  • Basic Life Support (BLS) - AHA American Heart Association or American Red Cross within 60 days of hire

Preferred

  • 3 years of relevant experience work experience preferred

  • Knowledge of community resources; social, health and public agencies and services.
