Josselyn Behavioral Health Care Coordiantor in Grayslake, Illinois

Job Summary

The Behavioral Health Care Coordinator is responsible for overall introductions and coordination of care between partnering facility and The Josselyn Center's services, including: intake, assessment, case management, therapy and psychiatric evaluation, medication monitoring, and supported employment.

Essential Responsibilities and Metrics

  • Provide daily proactive outreach and care coordination between assigned partner facility and The Josselyn Center's services

  • Provides high-quality service and maintains 50% of direct service hours per month

  • Complete Service Ticket/Progress Note within three working days of day of service and at the end of each week

  • Support and closely coordinate behavioral health treatment

  • Track client follow-up, treatment adherence and clinical outcomes. Document client encounters and re-engage clients who are not participating in their treatment

  • Develop, modify, and review treatment plans

  • Achieve metrics established to avoid client hospitalizations

  • Provide counseling and/or case management for highest acuity clients

  • Crisis intervention

  • Coordinate with clinical staff to monitor ongoing treatment for all clients referred by partner agency

  • Client centered consultation with community providers

  • Attendance and participation at clinical presentations and staff meetings

  • Use of supervision or consultation when required or needed

Qualifications

  • Master's level Health Professional

  • LCSW/LCPC Preferred and/or a minimum of 2 years related experience, specifically in crisis care setting

  • Three professional references that reflect ability to perform clinical work

  • Displays high energy, empathy, and organizational skills daily

  • Work flexible schedules, including nights and weekends when necessary

  • Experience supporting underserved, transient populations with high behavioral health care needs and co-occurring physical health challenges

  • Experience identifying and evaluating behavioral health disorders

  • Fluent English/Spanish preferred
