Job Summary The Registered Nurse II at Prevea plays a crucial role as an integral member of our clinic team, delivering patient-centered care. They provide patient care, educate patients and their families, and collaborate with other healthcare professionals to ensure that patients receive the best possible care. The Registered Nurse II delivers direct patient care by identifying patient care needs through encompassing a range of nursing assessments, procedures, and medication administration. Additionally, a Registered Nurse II may also provide phone advice or triage services as needed. The Registered Nurse II is accountable for the delivery of exceptional and safe care within the scope of practice as defined by the Wisconsin Board of Nursing and Prevea\'s policy. Through collaborative practice with members of the Prevea team, they are responsible for patient outcomes that meet the high quality of care provided by Prevea. What you will do Provides direct patient care which includes, but it is not limited to the rooming of patients, obtaining medical information, performing comprehensive initial patient assessment and reassessment pertinent to specialty area of practice, administer medications and monitor for reactions or side effects, and maintains patient charts/records. Follow nursing process of assessment, planning, intervention, and evaluation. Communicates information with patients, physician, advanced practice providers and staff to ensure quality and continuity of care. Assists the physician or advanced practice provider with procedures, notes their instructions/orders and makes necessary arrangements to ensure that they are carried out. Communicates relevant clinical information to the care team, physicians, and/or advanced practice providers to optimize patient outcomes. Triages patient, family, external provider, or affiliate staff\'s concerns/inquiries. Manages patient/family health concerns through telephone triage. Actively seeks information to understand patient circumstances, problems, expectations, and needs. Facilitates caller\'s access to appropriate level of care by assessing needs and offering clinical options using medial judgement within scope of licensure.
