GENERAL DESCRIPTION

Performs complex (journey-level) social services work. Work involves serving as a liaison between clients and their families in obtaining assistance and rehabilitative treatment. May train others. Works under general supervision with moderate latitude for the use of initiative and independent judgment.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Demonstrate high ethical standards, trustworthiness, and personal integrity. Regular attendance, dependability, and promptness are required to provide quality and consistent services to ECI children and families.

Interact effectively with all customers, internally as well as externally. Willing to accept coaching and constructive criticism from others while developing effective working relationships with all team members. Support team concepts and cooperate with others to achieve team objectives.

Complete the intake process for assigned referrals within 7 working days of the referral date 100% of the time or document all barriers encountered that prevent meeting this timeline. The intake process includes conducting the phone interview to establish concerns and sending the parent information by mail or email to the family.

Schedule children for evaluation or IFSP (medical qualifiers) with appropriate evaluation teams in order to meet the 45-day timeline 100% of the time or document all barriers encountered that prevent meeting this timeline.

Discontinue referrals that are parent declined or could not contact-completing all documentation to support efforts to contact.
