Mount Sinai Rehabilitation Hospital is looking for a Case Manager to join our Rehabilitation Case management Team.

The Rehabilitation Case Manager promotes continuity of care and effective appropriation of services for rehabilitation patients of all ages, particularly focusing on adolescent, adult, and geriatric patient. The case manager follows the process of planning, organizing, coordinating, implementing, and monitoring the services and resources needed to respond to an individual’s healthcare needs related to rehabilitation. The case manager is the key liaison for patient, family, and interdisciplinary team members to achieve quality, cost-effective outcomes, in addition to providing communication between payor sources and outside reviewers.

You Will Be Responsible For :

  • Consults with treatment team, health care providers, clients, and/or family members to facilitate problem solving, treatment planning and education. Provides appropriate feedback regarding client function and plan to patient, family, treatment team and other professional in a clear and concise manner.

  • : Provides counseling and support services to clients and family members and ensures that needs are addressed. Involves families in decision-making.

  • Coordinates services for clients with referring agencies; facilitates discharge planning; assists with referrals to community providers for support, education and any necessary additional services

  • Maintains and improves clinical competencies via continued education, professional development and program enhancement. Provides formal program information to staff and community, and participates in program development.

  • Participates and contributes to multidisciplinary patient care conferences, committees, team meetings, etc.

Requirements :

  • Master’s degree of accredited school required - MSW, LCSW, PT, OT, RN

  • License Required

  • 5 years acute healthcare setting experience, 3-5 years rehabilitation experience preferred, 3-5 years’ experience in managed care environment preferred.

  • Ability to communicate effectively both orally and in writing. This may include explaining clinical, technical and diagnostic procedures to clients and their families, counseling clients and their families and enforcing policies and regulations in difficult situations.

  • Work requires the analytical ability to collect information from diverse sources, apply professional principles in performing various analyses, and summarize the information and data in order to solve problems or design complex systems and programs.
