Trinity Health Clinical Pharmacist - Hospital Inpatient in Hartford, Connecticut

Description:

Position Purpose

Ensures the safe and effective use of pharmaceuticals through their proper storage, handling, evaluation, preparation, compounding, labeling, dispensing, and administration, in conjunction with state and federal regulations and departmental policies. Maintains proper records. Follows department guidelines with regard to clinical interventions and provides drug information as required and /or requested.

What you will do

  • Prepare, dispense, and supervise the preparation of all medications and IVs for patient use in the hospital

  • Advise other health care professionals on drug interactions, incompatibilities, side effects, and proper administration of medications

  • Supervise non-pharmacist personnel by assigning duties, checking all orders, maintaining efficient workflow, and checking IVs and other medications prepared by technicians

  • Assess need for non-formulary items ordered according to department policy, and initiates contact with prescribing physician for substitute.

  • Verify physicians’ orders for proper dose, regimen, allergies, potential drug-drug interactions/incompatibilities, adverse drug reactions and monitor lab data on all patients before dispensing medication

  • Verify all narcotic orders are received, maintain perpetual inventory log, perform weekly C-II inventory and monthly controlled drug inventory and ensure all forms are completed and filed appropriately

Minimum Qualifications

  • Pharm D degree and completion of a PGY-1 Pharmacy residency plus 3 years of relevant experience. If no residency training, 5 years of post-graduate inpatient hospital experience in a clinical setting with experience in decentralized services

  • Board Certification or acquisition of Board Certification in next 2 years is desired

  • CT Pharmacist license

  • Counsel and educate patients and their families, communicate effectively with physicians and other health care professionals, assign and follow-up on the work of other employees

  • Provide direct patient care to neonate, pediatric, adolescent, young adult, adult and geriatric patients

Position Highlights and Benefits
