Position Purpose:

As a Speech Therapist at Trinity Health Of New England At Home, you will provide professional speech and audiology diagnostic and therapeutic services to agency clients as authorized by their physician. This position offers a $5,000 sign-on bonus and ensures compliance with the state's professional practice act for speech pathologists, along with meeting all applicable licensure and certification requirements.

Key Responsibilities:

  • Client Assessment & Planning: Accurately assess clients at admission and required time points using standardized documentation. Develop a treatment plan in collaboration with the physician and client, focusing on achievable goals to improve or stabilize the client's condition.

  • Visit Prioritization: Effectively prioritize and plan visits based on client capabilities, condition, physician orders, and available resources.

  • Regulatory Compliance: Maintain a working knowledge of Federal, State, and local laws and regulations, including the Organizational Integrity Program, Code of Ethics, and agency policies. Adhere to these standards with honesty, ethics, and professionalism.

  • Care Coordination: Make appropriate referrals to other disciplines and services. Coordinate care to ensure effective and efficient service delivery.

  • Intervention & Education: Implement preventive, therapeutic, and rehabilitative procedures according to the treatment plan. Instruct clients and their families on methods to improve communication, swallowing, or other deficits.

Minimum Qualifications:

  • Certificate of Clinical Competence in Speech Pathology (CCC-SLP) from the American Speech and Hearing Association, or working toward certification with required supervised experience.

  • Licensed to practice as a Speech Pathologist in Connecticut.

  • Ability to function with minimal direct supervision, maintaining a professional relationship with clients and their families.

  • Valid Driver’s License and reliable transportation.
