JOB SUMMARY

The Latino Outreach Case Manager is responsible for recruiting and engaging program participants throughout the Hartford community. Works in coordination with other agency and community organizations to develop a pipeline for referrals. Conducts outreach and individualized case management for Latino members of the community in need of recovery supports, treatment, and/or withdrawal management resources. Addresses biopsychosocial needs as appropriate to eliminate barriers to connection to care.

KEY RESPONSIBILITIES

  1. Engages clients in the community, discuss clients health-related social needs, and offer individualized resource/referral services.
  2. Provides linkage and navigation assistance for clients and their families to community services.
  3. Helps clients and their families identify and access other Village clinical services and/or other community resources. Screening will provide the basis for referrals, and navigation services will include advocacy and follow-up.
  4. Participates in educational programming and individual services to clients and families to provide information about recovery, addiction, use of Naloxone, and other relevant topics.
  5. Develops and maintains collaborative relationships with other community agencies, organizations, and referral sources to provide comprehensive and effective treatment services for clients.
  6. Maintains effective, positive working relationships with referral sources.
  7. Meets all standards for timeliness and accuracy in performing data and reporting functions, including communication with referral sources.
  8. Produces and communicates timely and accurate progress reports, as appropriate.
Experience and Skills

EDUCATION

  • Bachelors degree preferred, will consider experience in lieu of some formal education.
  • High School Diploma/GED required.

EXPERIENCE/QUALIFICATIONS

  • At least one year of related case management experience.
  • Must be culturally competent to serve diverse client population; bi-lingual (English/Spanish) fluency required.
  • Demonstrated extensive knowledge of recovery-based supports and have the ability to integrate these resources into treatment.
  • Ability to communicate comfortably with clients, family members and significant others, and community resources and professionals about recovery-oriented topics.
  • Have a working understanding of mental health disorders and trauma and how they can impact outreach, engagement, treatment, and recovery.
  • Maintain an unbiased, working knowledge of medication assisted treatment formulations as a tool to support recovery.
  • Ability to professionally document all relevant client information and maintain accurate record of services provided.
