Fairview Health Services Behavioral Health Clinician LICSW in Hibbing, Minnesota

Fairview Range is hiring a Behavioral Health Clinician (LICSW) for our Inpatient Behavioral Health Department in Hibbing, MN.

As a valued member of the primary care team, the Behavioral Health Clinician plays a crucial role in leading the overall healthcare needs of the enrolled patient population. The clinician's primary functions include assessment, consultation, and direct care related to the prevention, identification, treatment, and ongoing management of psychosocial/behavioral concerns impacting the overall health of the defined patient group. Embedded within an integrated and consultative interdisciplinary team, the Behavioral Health Clinician employs a primary care approach to address the behavioral/psychosocial aspect of both routine and complex medical conditions. This role involves providing consultation to the medical team passionate about preventing, identifying, treating, and managing behavioral health/psychosocial issues and conditions. Population-based healthcare is delivered through individual, group, and community-based services applying evidence-based behavioral interventions tailored to identified medical conditions.

Schedule: This position will work 80 hours per two weeks. The shift length will be 8-hours on the Day shift. Weekends: None

As a Behavioral Health Clinician, you will be responsible for:

  • Conducting assessments and evaluations of behavioral health conditions by means of diagnostic and screening tools, clinical interviews, and consultation with the medical team.

  • Contributing to the development and implementation of treatment plan in collaboration with the patient and primary care team.

  • Providing primary behavioral health interventions for identified mental health and substance use conditions.

  • Maintaining concise, timely, and accurate documentation in the patient’s medical record.

  • Understanding of integrative health care in terms of the interactive and interdependent expressions of the physical/biological, emotional and psychological dimensions of health.

  • Enter, update, and review the patient registry to conduct panel management. Fulfill expectations for how to use and maintain the patient registry.

  • Connecting patients with community resources as needed to meet basic living and support needs.

  • Developing and nurturing relationships with other community and social support providers to aid in effective referrals and timely access to services.

  • Responding to the needs of the Behavioral Health Home Team; providing expertise, direction, consultation, and leadership to the Community Health Worker and Social Work Care Coordinator.

Required Qualifications

  • LICSW – with the ability to be credentialed through Fairview.

  • 1 year prior mental health experience.

  • Knowledge of completing diagnostic assessments.

  • BLS

Preferred Qualifications

  • 2 years prior mental health experience.

  • Experience working in a primary care setting, Crisis counseling.
