As a Medical Social Worker, You'll Make a Difference By:

  • Ensuring all activities align with Elara Caring’s vision and leadership.

  • Assessing patients to identify psychosocial, financial, and environmental needs.

  • Making initial social work evaluation visits and reevaluating needs during each visit.

  • Communicating significant findings and changes to supervisors, physicians, and other care personnel.

  • Reporting unsafe conditions and visit outcomes by the end of the day.

  • Implementing safety plans using patient, family, and community resources.

  • Participating in the development of the Plan of Care to ensure quality and continuity of care.

  • Verifying the Plan of Care and providing care according to established standards.

  • Revising the Plan of Care in response to patient care issues.

  • Writing physician orders for additional visits and changes to the Plan of Care.

  • Incorporating patient care goals in the clinical notes.

  • Performing skilled services/interventions in accordance with accepted standards.

  • Counseling and instructing patients and families to meet social work-related needs.

What is Required?

  • Master’s Degree or Doctoral Degree in Social Work from a school of Social Work accredited by the Council of Social Work Education

  • 1+ year of social work experience in a healthcare setting.

  • Current State License as a Licensed Social Worker (LMSW/LiMSW)

  • 50% travel requirements to see patients

  • Valid driver's license and car insurance

  • Excellent verbal and written communication skills
