Catholic Charities of LI Medical Care Counselor in Huntington, New York

CCLI is looking for Medical Care Counselors with generous hearts and a desire to contribute to the well-being of others to carry out our mission on Long Island. By joining our team, you will share in the joy of providing care with dignity and life with hope to our developmentally disabled group home residents. The MCC supports and provides health-related advocacy for residents.

Responsibilities:

Provide continuous protective oversight of the individual Schedule, transport and accompany individuals to appointments with providers; facilitate communication with providers re: individuals health changes Maintain all health-related documentation, including lab work results, consultation reports and updated medication prescriptions Communicate daily with nursing and residential management re: individuals medical status and treatment plans Manage medication inventory including prescriptions, over-the-counter medications, nutritional supplements and first aid supplies Provide companionship visits to individuals that are hospitalized, as required.

Qualifications: H.S. diploma/GED Valid NYS Drivers License Experience working with developmentally disabled adults strongly preferred C.N.A. or equivalent training helpful Ability to transfer and transport individuals to/from medical appointments in all types of weather, using house passenger van Detail-oriented, with well-development organizational and interpersonal communication skills Willing and able to provide compassionate, person-centered care
