Title of Position: PHD & NCCDC Housing Counselor

Immediate Supervisor: Housing/Payee Director

Areas of Responsibility:

  1. Responsible for coordination and implementation of PHD (Permanent Housing for the Disabled), and provide Case Management services for NCCDC Clients.
  2. Responsible for determining eligibility, and to schedule and complete appointments for clients seeking services through the program.
  3. Responsible for service delivery, information and referral to all ICCAP services, and linking Participants to any/all community service programs that they're eligible for.
  4. Responsible for client files in in accordance with program and agency requirements.
  5. Responsible for tracking program Participants' demographics on a monthly basis.
  6. Responsible for the completion of reports as required.

Specific Duties:

  1. Establish linkages with programs, and refer clients to services under the ICCAP umbrella as well as to services provided by outside agencies, follow-up to assure receipt of services.
  2.  Determine program applicant's eligibility for each program; income, disability, veteran, and need for project's participation, through a comprehensive household need assessment.
  3. Complete all necessary client information forms and obtain appropriate documentation for the files.
  4. In conjunction with clients, identify major problem areas, develop a goal plan to impact those problems, and establish a time frame to meet goals.
  5. Maintain complete and accurate case files; entering intake information case notes, goal plans participants' progress, etc. through the agency client database, and HMIS in a timely fashion.
  6. Conduct as needed one-on-one interviews with program participants via phone or in person to follow-up on goals, referrals, and other needs.
  7. Visit the participants' residences at least once a month for on-site interview, and housing inspection (PHD).
  8. Maintain strict confidentiality; obtain releases and work closely with the community contacts (Case Managers, Agencies, Landlords, family members, etc.)
  9. Summarize project data; generate reports, etc. as required by ICCAP, NCCDC, and U.S. Department of Housing and Urban Development.
  10. Complete monthly rent determinations and forward the results to the Housing/Income Management Director.
  11. Schedule annual apartment inspections with ICOPD
  12. Work as a team and maintain a positive attitude.
  13. Other duties as assigned.

     Screening Criteria:

  14. Must have self-motivation to complete daily tasks and procedures without close daily supervision.

  15. Ability to ensure the integrity of data entered.
  16. Willingness to work with team members to achieve desired outputs.
  17. Strong communication skills, both verbal and written.
  18. Good organizational skills and have the ability to meet deadlines in a timely manner.
  19. Maintain accurate and complete case files, records and reports.
  20. Must have working knowledge of computers, including MS Word, Excel, and be willing to learn and utilize agency software programs such as ORS, and others as they pertain to your programs.
  21. Must have a minimum of one year of counseling experience and/or training in related fields, with demonstrated ability to work on behalf of low-income clients.
  22. Knowledge of Indiana County's social service network and community resources.
  23. Must have a valid PA driver's license, reliable transportation and be willing to travel in Indiana County and other counties if required.  
  24. Must have vehicle insurance to comply with agency standards of coverage.
  25. Must have or be willing to obtain Motor Vehicle Report, Act 33 & 34 clearances, and FBI clearance with fingerprinting.
