Responsibilities

As a Social Worker II, you will provide emotional support, crisis intervention, and counseling to individuals across all age groups, including pediatrics, adolescents, adults, and older adults, along with their significant others. Your role involves helping clients navigate a range of social and emotional challenges, offering information and referrals, and connecting patients and families to financial assistance and supportive services related to hospitalization, illness, disability, or trauma.

Key responsibilities include:

  • Conducting case management and milieu management.

  • Performing comprehensive psychosocial assessments.

  • Assisting in the development and ongoing evaluation of treatment plans.

  • Collaborating as part of a multidisciplinary team to coordinate discharge planning.

  • Ensuring accurate documentation and maintaining clinical records for each client throughout their stay.

Your expertise will play a vital role in supporting our clients and enhancing their overall well-being.

Qualifications

Education and Work Experience

Required qualifications:

  • Masters Degree in Social Work related field OR.

  • Masters Degree in Marriage and Family Therapy or related field.

  • In states without required licensure, then current registration with State Board is required within 3 months of hire. In states with required licensure, license required at time of hire and must be maintained.

  • Behavioral Violence Prevention Training within 3 months of hire and maintain current.

  • Minimum one (1) year hospital or Outpatient experience with emphasis in mental health care.

  • Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, disability, chronic illness, etc.).

Preferred qualifications:

  • Current BCLS (AHA) certificate upon hire and maintain current preferred.
