Job Summary

The Nurse Navigator ensures prompt patient care and acts as primary point of patient entry. They collaborate with the healthcare team to meet patient needs, organize care, and review requirements. The successful applicant will work autonomously and collaboratively to advocate, educate, and coordinate for patients. Good communication skills required.

What You'll Do

  • You'll coordinate with health professionals to conduct necessary tests for clear diagnoses. Ensuring patients have smooth access to top-quality healthcare at all stages of treatment.

  • Collaborating with doctors and providers is vital. Education for patients and families is essential. Include diagnoses, treatments, and services. Your role is crucial in patient care.

  • Your role will require an exemplary level of responsibility including decision-making and delegation to ensure safe, professional patient care.

  • Regularly visit physicians and staff. You will be a liaison between them and patients. Provide information about our facilities and general guidance to patients and families.

  • You may need to help patients and families navigate the hospital and provide regular follow-ups.

  • Communicate effectively, both orally and in writing

  • Organise tasks and work efficiently, as it's vital in evaluating the needs of patients and executing care plans.

  • Navigate through the complexity of the healthcare system, acting as a go-between and advocate for patients throughout their healthcare journey.

Qualifications

  • Bachelor degree.

  • You should have at least 3 years of relevant experience.

  • License Registered Nurse.

  • Basic Life Support should be acquired within the first month of being hired.
