Lancaster General Health Resource Coordinator - Case Management (40 hours/week) in Lancaster, Pennsylvania

POSITION SUMMARY:

Assists through facilitating patients through the continuum of care and arranging transition needs under the direction of the Case Manager/Social Worker.

HOURS: Full-time, hours vary from 7:30AM - 5:00PM. Will work every 7th weekend from 8:00AM - 4:30PM (could vary). Rotate with team to work designated holidays (Martin Luther King, Memorial Day, Juneteenth, Independence Day and Labor Day) - subject to change. No on-call hours. Opportunity to work a limited hybrid schedule after 6 months.

ESSENTIAL FUNCTIONS: Qualified individuals must have the ability (with or without reasonable accommodation) to perform the following duties:

  • Assists department staff in facilitating patient transitions of post-acute providers and other community resources as requested, including completion of referral and authorization processes as appropriate.

  • Maintains professional relationships with a wide variety of community agencies by facilitating communication and information between clinical personnel and post acute providers.

  • Documents appropriate information on medical records regarding the transition plan. Assists with the coordination and communication of the discharge process.

  • Participates in reimbursement, certification and authorization related activities as directed.

  • Documents payer and authorization information as needed.

SECONDARY FUNCTIONS: The following duties are considered secondary to the primary duties listed above:

  • Interacts with patients and families, as needed, to coordinate post acute services safely and efficiently.

  • Other duties as assigned.

JOB REQUIREMENTS

MINIMUM REQUIRED QUALIFICATIONS:

  • High school diploma or equivalent (GED)

  • Two (2) years of clerical, customer service, or administrative support experience in a highly customer-oriented organization.

  • Two (2) years experience with basic keyboarding, personal computer use, and other office setting equipment.

  • Demonstrated ability to be self motivated and to prioritize tasks efficiently and accurately.

  • Demonstrated ability to work as a member of a team.

  • A health care provider in good standing with Medicare, Medicaid, and other federal and state health insurance programs, i.e. not excluded from participation in Medicare, Medicaid or any other federal or state health insurance program.

PREFERRED QUALIFICATIONS:

  • Two (2) years experience as a Patient Care Assistant or LPN.

  • Two (2) years experience in a hospital/health or insurance/payer setting.

  • Prior payer experience.

  • Prior logistics experience.
