Job Summary:

Under the supervision of the Women’s Clinic Administrator, the Community Health Worker will support patient navigation efforts by providing telephonic and in person support to patients who have identifying barriers to accessing health care, providing social support and health education. The Community Health Worker will serve as a liaison/intermediary between individuals, communities and health and social services to facilitate access to care, improve the quality and cultural responsiveness of service delivery, and address social determinants of health. Works in both clinical and community-based settings, to include client's homes.

Core Competencies:

  • Experience working with Medicare, Medicaid or Special Needs populations.

  • Ability to connect with people and understand the challenges they face.

  • Ability to use a range of outreach methods to engage individuals and groups in divers settings.

  • Well connected to the community and resources within the community they will serve.

  • Effective written and verbal communication skills demonstrating respect and cultural awareness during interactions with patients.

  • Ability to travel throughout the assigned region and comfort with conduction home visits.

  • Fluent written and verbal skills in English and Spanish, preferred.

Job Requirements:

  • High school degree or equivalent. 

  • Be able to obtain Community Health Worker Certification within first month of employment, if not currently certified as a Community Health Worker. 

  • 1+ years of experience in a physician's office or clinical setting. 

  • Proficient computer skills to work efficiently with electronic medical records.
