Trinity Health Full-Time Intake Coordinator in Livonia, Michigan

  • Must be within a 50 mile radius of Trinity Health At Home location

  • Work shift Monday through Friday 12:30pm – 9:00pm

  • Will work a weekend rotation, one every 6 weeks

  • Will work a holiday rotation, two per year

Position Purpose:

The Customer Care Center (C3) Intake Coordinator is responsible for communicating, coordinating, and documenting care for the transition of patients to home-based services. This role is also responsible for identifying / determining appropriateness of care, gathering medical, gathering /verifying insurance and financial data, documenting in the EMR, and communicating with patients and families as well as working with Care Management, physicians, and referral sources.

The C3 Intake Coordinator is often the first point of contact for referral sources and people referred for Home Based Services. This role requires a high-level skill set for screening and referral triage, customer service, critical thinking, and the ability to navigate multiple applications and computer systems.

What You Will Do:

  • Responsible for determining appropriateness of care, referral screening and acceptance based on THAH guidelines.

  • Accountable for gathering /verifying insurance and financial data from multiple referrals sources and patients.

  • Responsible for accurate documentation in the EMR and manages the referral from start to finish ensuring minimum data set is obtained and verified.

  • Accountable for coordinating and documenting care types, order details and all required information for the safe transition of patients to home-based services.

  • Ensures referrals are being managed with the highest level of customer service, verifies, and communicates accurate billing and demographic information, documents correct and accurate information in EMR for billing purposes.

  • Demonstrates high level computer skill so that the referral is transmitted in a timely manner to the appropriate agency contact for efficient scheduling.

  • Meets and exceeds the set productivity standards for referral management completion and demonstrates ownership for follow-up needed for referral management to ensure patients are seen within established guidelines

  • Pay rate $17.87 - $22.00 per hour with shift differential pay.

Minimum Qualifications:

  • Associate degree preferred; High School diploma required with equivalent 2-4 years industry experience.

  • 2-3 years home care, hospice, discharge planning, or case management experience preferred.

  • Working knowledge of governmental reimbursement criteria. Knowledge of third party/managed care contracts related to THAH services preferred.

  • Microsoft computer skill set required. Experience with THAH current software platforms and applications preferred.

  • Working knowledge of standard office equipment and remote technology required

  • Must be able to communicate with high-level customer service, phone etiquette and confidence to effectively work with internal and external customers.

  • Demonstrates a commitment to customer service and demonstrates agility and challenges to engage and participate in process improvement.

  • Able to consistently demonstrate commitment to the mission and organizational code of ethics and exhibits Behaviors of Excellence.

  • Must possess interpersonal skills sufficient to interact effectively with clients and their families, as well as caregivers, peers, subordinates, supervisors, referral sources, divisional personnel, and the public.

  • Previous experience working in a healthcare environment, demonstrating computer literacy.
