UCLA Health Community Health Worker Supervisor - Social Drivers of Health Program (Flexible-Hybrid) in Los Angeles, California

In this role, you will oversee the daily administrative and operational functions of the Community Health Workers (CHW). You will carry a limited caseload and support patients and their social and health needs to navigate and access community resources, social services, and medical systems. As Supervisor, Community Health Worker (CHW), you will travel to the UCLA Health practices, community-based organizations, patients homes, etc. as needed to provide support to CHWs, Clinical Advisors, patients, caregivers, physicians, office staff and other key stakeholders have the support they need and that CHWs are performing at the highest level of their skill sets according to their job description, standard work, competencies and policies. You will also collaborate with the Op.. Manager, Social Care and the clinical leadership of each site to ensure member satisfaction and operational efficiencies.

As Supervisor, you will be responsible for assisting UCLA Health social care team members and patients to access, navigate and maintain needed social care services both within and outside of UCLA Health. You will be a member of the Social Care team, part of Ambulatory Care Management and work under the supervision of the clinical and operations management in collaboration with other CHWs and Clinical Advisors. You will assist in partnering with community-based organizations, connect patients and their families to social drivers of health (SDOH) related services including but not limited to food, housing, transportation, employment, childcare, education, and mental health services. You will also support staff in patient social care needs assessments, advocate for patient preferences and resources, assists in the identification of needs and resources, referral, outreach to relevant community partners, and assist the team in designing and implementing educational efforts.

*This position is 25% flex-hybrid and includes significant field work (patient homes, community and office visits with patients) as well as administrative support to assist the team in bridging clinical and social care.

Salary Range: $68,800 - $141,000 Annual

QualificationsRequired:

  • Community Health Worker Certificate

  • Minimum 1-2 years of Community Health Worker Certification and Field Experience

  • Knowledge of the public healthcare environment, Social Drivers of Health (SDOH) and social care resources in UCLA Health service areas.

  • Excellent communication skills; verbal, written and interpersonal. Prior experience with population management and care coordination required.

  • Prior experience patient centered medical home implementation or operations required, including knowledge of URAC and NCQA guidelines for patient centered medical home.

  • Proven experience with insurance eligibility checking, prior authorizations, referrals, and orders for both commercial and government payors.

  • Skill in analyzing information, problems, situations and procedures to define objectives, identify pattern and formulate conclusions.

  • Ability to creatively integrate competing demands of an outpatient setting into a productive working environment.

  • Ability to interact diplomatically and sympathetically with a patient population in varying degrees of health and effective communication with staff, faculty and the general public in conveying information to varying organizational levels

  • Proven experience communicating difficult information to physicians and leadership and relaying nuanced information in a diplomatic and productive manner.

  • Skill and experience in developing Quality Improvement action plans, including conducting case studies and root cause analyses.

  • Ability to adapt and implement frequently changing procedures.

  • Knowledge of medical terminology.

  • Knowledge of health system registration procedures.

  • Ability to be flexible with working hours given adequate notice and ability to work weekends are required by the assigned practice(s)

  • Superior analytical and problem solving skills.

  • Strong leadership skills and independent decision making ability.

Preferred:

  • Ability to speak and write in a language other than English (e.g., Spanish, Korean, Mandarin, Tagalog, Armenian, American Sign Language, etc.) is strongly preferred.

  • Training and experience with diverse patient populations including but not limited to those who experience homelessness, low SES, limited English proficiency, and others underrepresented groups.

  • 1-2 Years Supervisory Experience Strongly Preferred.

  • Bachelor's preferred but not required.
