Description

Job Purpose : This professional position serves as the case manager and service provider to individuals participating in Employment and Community Services programming. The Case Manager is responsible for maintaining the client record, communicating with stakeholders, and coordinating the overall care of the person served. The Case Manager also promotes the importance of community integration and inclusion for all through education and service delivery.

LIFE Services population:individuals with intellectual/developmental disabilities and medical support needs.

Brain Health Services population: individuals with traumatic and acquired brain injuries.

Reporting Relationships

This positionreports to theLIFE or Brain Health Program Manager based on program assignment and has no direct reports.

Essential Job Functions :

  1. Completes all required documentation, meeting established standards and timelines, to uphold the integrity of the client record. Shares pertinent information regarding plan of care and with the team, directs service delivery and monitors progress and goal achievement.

  2. Maintains effective and supportive relationships and timely communication with stakeholders, including coordinating biannual plan of care conferences.

  3. Ensures proper authorization is in place prior to service provision and thereafter for length of stay.

  4. Provides a safe, therapeutic learning environment for all persons served. Programming is focused on enhancing growth and independence through teaching and promoting personal choice, identifying interests and skills sets and developing general ability and function pertinent for community engagement and desired life activities.

  5. Responsible for assisting individuals at mealtime to ensure safety and promote a pleasant and respectful eating environment.

  6. Collects pertinent data necessary for metric and outcome reporting.

  7. Mentors Direct Service Professionals and Program Support Professionals in classroom instruction, engaging persons served, adapting activities, and increasing level of comfort when running groups in the community. Demonstrates professionalism and encourages teamwork.

  8. Creates new or supplemental components to existing curricula that are relevant, engaging, appeal to different learning styles and modalities, and meets the needs of the individuals served.

  9. Identifies and introduces assistive technology, accommodations, compensatory strategies, or support plans as needed to improve client presentation and/or performance.

  10. Represents MCFI and advocates for people with disabilities through formal and informal participation in professional and community organizations and interactions with community partners.

  11. Practice and promote safety in the workplace; reports all accidents according to agency procedures.

  12. Perform other related duties as assigned.

Qualifications

Required Technical Skills, Experience, Education, and Credentials :

  1. A bachelor’s degree in Community Counseling or Rehabilitation, Psychology, Social Work or another related field.

  2. Knowledgeable of the physical, psychological, and social aspects of traumatic and acquired brain injuries and the resultant impact on the individual and family.

  3. Knowledge of case management principles and practices.

  4. Knowledge of community integration and inclusion best practices.

  5. Must have strong oral and written communication skills and demonstrate computer literacy.

  6. Skill and ability to interpret observational data to make objective and impartial written/verbal reports.

  7. Must be comfortable conducting formal classroom sessions and apply various teaching methodologies to meet the needs of the people in programming.

  8. Understanding of behavior modification techniques, compensatory strategies, and accommodations.

  9. Current First Aid certification or completed within first 90 days of hire.

  10. Current Non-Violent Crisis Intervention certification or completed within first 90 days of hire.

  11. Current negative TB test.

  12. A personal vehicle, a valid Wisconsin driver’s license and insurance in conformity with agency limits.

  13. Ability and willingness to drive company vans for community-based activities.
