UW MEDICAL FOUNDATION Nurse Practitioner / Physician Assistant in MADISON, Wisconsin

JOB REQUIREMENTS: Nurse Practitioner / Physician Assistant Palliative Care - 31793 DESCRIPTION/RESPONSIBILITIES: Work Schedule : 100% FTE, days. Monday - Friday, eight-hour days between the hours of 8:00 am and 5:00 pm. No weekends, holidays, or on-call. You will work at East Madison Hospital in Madison, WI. We are seeking a Nurse Practitioner or Physician Assistant to: * Provide complex symptom management recommendations. * Work in collaboration with the Palliative Care Medical Director, attending medical team, and Palliative Care Team members to deliver palliative care to patients in the acute care setting with life-limiting chronic diseases. * Effectively communicate with patients and family by utilizing concepts of motivational interviewing. * Perform assessment process to include patient history, physical, psychosocial, and spiritual assessment. * Provide support, counseling, and information to patients and families dealing with end-of-life issues or with issues related to life-threatening or serious and complex illness. * Facilitate patient/family conferences as related to palliative care or hospice. If Physician Assistant: Bachelor\'s or Master\'s degree from accredited PA Program. Licenses and Certifications : Minimum - State of Wisconsin NP or PA licensure If Nurse Practitioner*: * Active RN and APRN certifications and licenses, including prescriptive authority, in state(s) of practice AND * Active board certification as a NP in area of clinical practice.
