JOB REQUIREMENTS: Manitowoc County Human Services Department is seeking a CST Care Coordinator. The CST program is a family centered, community-based initiative that aids with the development of a coordinated system of care for youth with behavioral health needs. This position facilitates family team meetings and assists families in accessing resources to achieve their goals and: Provides care coordination for referred families. Conducts a structured standardized assessment to create a comprehensive and multi-dimensional summary of the strengths and needs of the child and family. Collaborates with team members to develop a Plan of Care for each family. Ensures that the plan of care also includes a safety plan for each child to address potential crisis situations. Completes all Plan of Care paperwork.
