*Case Manager - Behavioral Health - 1 * Marion; Monday -- Friday; 8 hours/day - may vary to meet client needs

Assist in the planning, development, and delivery of outcome-effective treatment. Maintains an ongoing working knowledge of agencies and community/emergency resources for child, adolescent, adult and geriatric clients to advocate for patients. Works directly with assigned patients, assess to determine needs and identified goals.

*Bachelor's degree in social work or related field of study preferred associate degree in social work or related field of study and relevant work experience. *At least 1 -- 2 years' experience *Must be 21-years of age *BLS certificate
