This Social Worker position works collaboratively with the Family Services team to maintain or establish family connections between children and youth in out-of-home care and their relatives and informal supports. This encompasses identifying supports for children, youth, and families through the Family Find and Engagement model, providing consultation and support around out-of-home care providers, reunification, and permanency planning. This position must work toward the efforts and support the supervisor and Family Services Division to ensure compliance with program objectives and applicable standards, mandates, rules, regulations, and statutory authority. The work is performed under the immediate direction of a Family Services Supervisor with additional oversight. ESSENTIAL DUTIES AND RESPONSIBILITIES: These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned. RESPONSIBILITIES A. ESSENTIAL FUNCTIONS 1. Utilize a family-centered, trauma-informed, strength-based, culturally sensitive, and equity, diversity, and inclusion lens through which children, youth, families, and placement providers\' needs are assessed. 2. Engage child, youth, and family in family find discovery tools at the time of out-of-home care placement. 3. Complete intensive Family Find initiatives for children and youth to foster connections with relatives and informal supports, explore placement options, and long-term support. 4. Utilize ecomaps, genograms, and other discovery tools and search engines to map and locate prospective supports and placement resources. 5. Conduct extensive research, investigation, and outreach in locating non-custodial parents/guardians and extended family members. 6. Use Social Work methods, principles, and interview techniques to establish rapport and build trusting relationships to obtain information and effectively work with the family. 7. Assist with permanency support plans for youth. 8. Utilize strong communication and engagement skills to collaborate with the team including community partners involved to obtain optimum benefit from the utilized services. Individual may use personal vehicle or other means of reliable transportation.
