Veterans Affairs, Veterans Health Administration HUD-VASH Social Worker (Program Coordinator)- Aging and Disabled Veteran Program in Martinez, California

Summary This position collaborates with local Public Housing Authority (PHA), community partners, and VA Medical Centers (VAMC), to oversee a new or existing HUD-VASH project-based development serving aging or disabled Veterans who can benefit from an enhanced model of care. This position will be a subject matter expert in aging and disabled Veteran care, and maintain and create new referral pathways to assist aging and disabled Veterans with independent living. This position will supervise and oversee program development, policy, and treatment coordination for the aging and disabled Veterans participating in the enhanced model of care, including administrative and clinical supervision of a multi-disciplinary team. This position will work on an interdisciplinary team to provide lead case management services and coordinate treatment plans. Reviews and Approves or Disapproves Leave Request. Assures that employees are trained and fully comply with the provisions of the safety regulations. Work Schedule: Full Time; Varying Schedule; Subject to change to meet the needs of the Agency Recruitment Incentive (Sign-on Bonus): Not Authorized Permanent Change of Station (Relocation Assistance): Not Authorized Pay: Competitive salary and regular salary increases When setting pay, a higher step rate of the appropriate grade may be determined after consideration of existing pay, higher or unique qualifications, or special needs of the VA (Above Minimum Rate of the Grade). Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66. Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement) Telework: Authorized; if approved, would be on a regularly scheduled basis but this is not a remote position. Qualifications Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. BASIC REQUIREMENTS: United States Citizenship. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Licensure. Must hold a full, current, unrestricted licensure as a Social Worker in a State, Territory or Commonwealth of the United States, or in the District of Columbia. Education. Posses a Master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). GRADE DETERMINATIONS:In addition to meeting the above basic requirements, applicants must possess the below grade requirements to be found qualified for this position. Education. One year of experience equivalent to the GS-11 grade level. AND, Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Knowledge, Skills and Abilities (KSAs). In addition to the above grade requirements, applicants must clearly demonstrate the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
