Loyola University Medical Center is seeking an experienced Advanced Practice Nurse (APN) to join our Outpatient Kidney Transplant team. The APN will provide specialized nursing care to kidney transplant patients, working independently and in collaboration with physicians, case managers, and the multidisciplinary care team.
Key Responsibilities:
Deliver advanced nursing care focused on healing, illness prevention, and reducing hospital readmissions.
Manage patient care, including developing individualized treatment plans, performing invasive procedures, and evaluating outcomes.
Adjust treatment plans as needed to meet patients' evolving health needs.
Coordinate complex patient and family education to support patient transitions between care units and improve care continuity.
Qualifications:
· APN
· Required: Masters of Science in Nursing
· Current State of Illinois RN and APN licensure
Certified as a Nurse Practitioner by the American Nurse’s Credentialing Center or American Academy of Nurse Practitioners
· Ability to qualify for Illinois and DEA prescribing numbers
· BLS and ACLS certification
