Trinity Health Clinical Dietitian II - Certified Diabetes Educator in Maywood, Illinois

Description:

Provides DSMT and MNT for diabetes patients in an outpatient clinic setting.

  • Use principles of teaching and learning theory and lifestyle counseling to help clients confidently and effectively manage their disease.

  • Provide individualized instruction for persons of all ages, incorporating cultural preferences, health beliefs and preferred learning styles of the client.

  • Conduct group diabetes education classes.

  • Provide diabetes self management training, which includes providing recommendations for nutrition, medication, and lifestyle changes.

  • Documents the assesment,planning, intervention and evaluation in the medical record following established guidelines.

  • Serves as a preceptor for dietetic interns.

  • Demonstrate leadership skills by presenting on nutrition topic related to their expertise.

  • Example of settings include journal club, in-service, Grand Rounds, THS-FANS program. This may also include participating on a hospital committee or THS-FANS sub-committee.

  • Participate in department activities to enhance patient satisfaction.

  • Keep current on nutrition research and incorporate into daily practice.

  • Other clinical duties as assigned.

Required: Bachelors Degree

• Preferred: Masters Degree

• Specify Degree(s): Dietetics, Nutrition or Family & Consumer Science with ACEND (Accreditation Council for Education in Nutrition and Dietetics) requirements completed

• Required: 1-2 years of previous job-related experience

Required: Certified Diabetes Care and Education Specialist
